The Ministry of Public Health and Social Assistance Department of Food Regulation and Control has implemented the new FS-DRCA-06 Form, through which the APPLICATION FOR REPLACEMENT OF THE SANITARY REGISTRATION OF FOODSTUFFS can be carried out.

FS-DRCA-06 form has been prepared so that users may request a new issuance of the previously issued document.

The requirements for this procedure are as follows:

  1. Complete the Application Form with the data indicated below:

a. Procedure typification:

– Indicate the registration number of the product

– Product expiration date

b. Product Holder:

– Name of the holder

c. Product Information:

– Product Name

– Product Brand

d. Reasons why a replenishment is required (explain in detail).

e. Signature of Owner or Representative

f. Company stamp

2. Payment voucher of the fee for the procedure (Receipt 63-A2):

To carry out this procedure at the Ministry of Health, the corresponding fee of Q.50.00 (USD$6.42) must be paid.
